Pedal positions
First check that there is 0% throttle actuation when the accelerator pedal is released. Then go on to check that when the brake pedal is released the brake signal is inactive. Check, if necessary, both pedal sensors and their wiring including connectors.

Diagnostic depth
The signals from the accelerator pedal sensor and brake pedal sensor have been implausible in relation to each other.
The accelerator and brake pedal have been operated simultaneously. This may be because the accelerator pedal is jammed or its return spring is broken. Under exceptional circumstances, a fault code may have been generated because of a fault in the wiring or because of a an internal fault in one of the pedal sensors.
